Healthcare AI Is Entering Its Platform Era
With 81% of physicians now using AI, health systems have moved past the adoption question. The new problem is what shared infrastructure makes scaling safe.
81% of physicians now use AI in their practice. That's where we are in 2026, according to the AMA Center for Digital Health and AI — more than doubling from 2023. The average physician runs 2.3 AI use cases in their daily workflow; three years ago, it was 1.1.
The adoption question is over. What health system executives are sitting with now is harder: how do we run this safely, at scale, across an enterprise with thousands of staff, multiple facilities, and regulatory exposure everywhere?
That's not a pilot question. That's a platform question.
The conversation has already changed
A few signals, taken together, tell you something is shifting.
Physician adoption is outrunning governance. The AMA data makes this explicit: clinicians are using AI faster than health system leadership anticipated, and organizational frameworks are struggling to keep pace (HealthLeaders). That's not a problem with physicians. They're solving real problems with the tools available. But organizations are accumulating AI usage they don't fully oversee, and that gap will eventually cost someone.
Meanwhile, pilots are stalling in exactly the wrong places. In pharmacy, where AI-driven clinical decision support should be mature by now, Becker's reporting shows that many AI programs still can't clear the pilot stage (Becker's Hospital Review). The pattern is consistent: a vendor deploys a tool, it shows promise in limited testing, and then it hits the wall of custom integrations, local validation requirements, and the absence of any shared infrastructure to carry it forward. Point-solution sprawl has a debt ceiling.
There's a funding signal too. AI is no longer treated as a standalone budget line in many health systems. It's being absorbed into broader IT and operational spend, the same category as EHR licensing and network infrastructure (Becker's Hospital Review). When organizations start treating something like infrastructure, they start expecting infrastructure-grade reliability and accountability. That expectation changes the conversation about how AI gets built and managed.
What platform thinking actually looks like
Sutter Health gave the clearest public case study at HIMSS26. The California system built a common AI infrastructure plugged directly into existing EHRs, imaging archives, and dictation tools. The goal was to deploy, monitor, and swap AI algorithms without rebuilding vendor connections from scratch each time (Healthcare IT News).
Three elements of their approach matter more than people realize.
The first is local validation. Sutter validates AI efficacy against its own patient population before any algorithm touches care workflows. Not vendor benchmarks — their patients, their data, their local conditions. That's the trust criterion that separates responsible enterprise AI from vendor trust disguised as due diligence.
The second is a governance committee with real authority. Not an advisory panel that gets briefed after deployment decisions are made, but a body that owns deployment decisions, KPI requirements, and evolving policies as models change over time. Governance committees are infrastructure, not compliance theater.
The third is replaceable model and vendor layers. This is underappreciated. If an organization has built its AI stack on a single vendor's proprietary architecture, swapping that vendor means rebuilding the plumbing. Fierce Healthcare put it bluntly in a recent piece: stop buying AI tools, start designing AI architecture. That framing is resonating because it describes where health systems are actually stuck.
Why the knowledge layer is next
Once a health system solves the infrastructure problem — shared deployment, local validation, real governance — the next failure point is what the AI reads.
Clinical AI at scale reads from document repositories: policies, clinical guidelines, formulary updates, safety protocols. Those documents get outdated. They conflict with each other. At the point-tool stage, that's manageable. One tool, limited scope, someone usually catches the bad output before it causes harm. At enterprise scale, with AI embedded across radiology, pharmacy, documentation, and clinical decision support, stale or contradictory source documents stop producing isolated errors. They produce systematic ones. The same wrong policy answer, delivered confidently, to hundreds of clinical queries a day.
ECRI named AI the No. 1 patient safety risk for 2026. Much of the safety debate has focused on model bias and hallucination. But in most health systems, the more immediate risk is the quality of the documents the AI retrieves from. A model can be accurate and still give dangerous answers if the knowledge base hasn't been maintained.
Shared AI infrastructure is only safe if what sits under it is current, consistent, and auditable. That means knowing which document version an AI retrieved when it gave a specific clinical answer. It means detecting when two policies contradict each other before a model has to choose between them. It means updating the knowledge base when guidelines change without relying on manual edits from staff running too thin to catch every conflict.

Can you validate any new algorithm against your own patient population? Vendor benchmarks are not a validation process.
If your current AI vendor raised prices by 40% tomorrow, what would it cost to replace them? If the answer is "multi-year rebuild," that's a dependency, not infrastructure.
When your AI retrieves from policy documents, do you know which version it used? Can you identify contradictions in your knowledge base before they surface as inconsistent clinical answers?
Does your AI infrastructure connect to your EHR, imaging, and documentation systems, or does each vendor maintain its own integration?
Healthcare AI is past the pilot stage in terms of adoption. What hasn't caught up is the architecture needed to make that adoption safe. Health systems that treat AI like infrastructure — shared, governed, locally validated, and built on a knowledge layer that gets maintained — will be in a very different position when the next wave of regulatory scrutiny arrives. The ones still running 47 disconnected tools won't be.
